Gynecomastia Surgery in Miami
A flatter, firmer, more masculine chest — with results that look natural, not corrected.
If you have puffy nipples, persistent chest fullness, or a chest that still looks enlarged despite working out, gynecomastia surgery may help. Dr. Daniel Careaga uses a customized full-chest contouring approach — designed to create a naturally masculine result, not just remove tissue under the nipple.

You've been working out. The chest still doesn't show it
For many men, the problem is not a lack of effort. Gynecomastia is enlargement of the male chest caused by excess glandular tissue, fat, or both — and it doesn't respond to diet or exercise. That's why men who are otherwise lean and fit can still struggle with puffy nipples, chest fullness, or a chest that simply doesn't look masculine. It's a structural issue, not a lifestyle one.
Does any of this sound familiar?
- Puffy or protruding nipples that don't go away with training
- Persistent chest fullness or softness despite being in good shape
- Avoiding fitted shirts, going shirtless, or being photographed
- A chest that looks disproportionate to the rest of your body
- Embarrassment or self-consciousness you've lived with for years
Full chest contouring — not just tissue removal
One of the biggest problems in gynecomastia surgery is focusing only on the tissue directly beneath the nipple. Traditional approaches that remove too much tissue under the areola without contouring the surrounding chest can leave behind a cratered or 'saucer deformity' — an indented, uneven result that still doesn't look natural. Dr. Careaga's approach treats the chest as a full contouring procedure.
What his approach includes:
- Full chest contouring — not just the area under the areola
- Gland and fat treatment — because many men need more than liposuction alone
- Small, strategic incisions to minimize visible scarring
- Natural-looking result — flat without a scooped-out appearance
- Skin tightening with Renuvion J-Plasma when loose skin is present
Renuvion J-Plasma — for skin that needs to contract
Removing gland and fat is only part of the result. The skin also has to tighten over the new chest shape. When loose skin is present, Renuvion J-Plasma helps tighten the tissue beneath the skin so the chest can contract more smoothly — helping the areola re-drape naturally and the chest look firmer and more defined.
A technique focused on shape, contour, and natural results
Patients choose Dr. Careaga because his technique goes beyond tissue removal. Every procedure is tailored to the patient's gland composition, fat distribution, skin quality, and chest anatomy. The goal is not just to remove what's there — it's to create a chest that looks flatter, firmer, and more athletic.

What patients say:
"I avoided shirts my entire adult life. Six weeks after surgery, I wore a fitted t-shirt for the first time. The result looks completely natural." — Patient review
Why the "around the areola" approach isn't enough
Understanding this distinction helps patients evaluate their options and choose the right surgeon:
|
Traditional Approach |
Dr. Careaga's Approach |
|
Focuses only on tissue under the nipple |
Contours the entire chest for a smooth, natural transition |
|
Risk of 'saucer deformity' — indented, uneven appearance |
Preserves a natural transition under the nipple |
|
Often ignores surrounding chest fullness |
Blends the inner and outer chest for a uniform result |
|
Skin laxity left unaddressed |
Renuvion J-Plasma incorporated when skin needs to contract |
The right approach treats the chest as a complete contouring procedure — not a spot treatment.
What recovery looks like — week by week
Recovery is generally manageable, and most patients are surprised by how quickly they return to normal activity.
Days 1–7 — Rest and compression
Swelling and some discomfort are expected. A compression garment is worn as directed. Rest is the priority during the first days.
Week 1–2 — Return to desk work
Most patients return to desk or non-physical work within 7 to 14 days. Light walking is encouraged. Compression is maintained.
Weeks 2–4 — Progressive recovery
Light cardio can begin around weeks 2–3. Swelling continues to improve. The chest contour starts to become more visible.
Week 4+ — Return to upper body training
Chest workouts and upper body training typically resume around week 4, based on healing progress and surgeon guidance.

FAQ
What is gynecomastia?
Gynecomastia is enlargement of the male chest caused by excess glandular tissue, fat, or both. It is not simply stubborn fat — dense glandular tissue does not respond to diet or exercise, which is why men who are otherwise lean can still have this concern.
Am I a good candidate for gynecomastia surgery?
Good candidates have puffy nipples or persistent chest fullness, are at a stable weight, are in good overall health, and have realistic expectations. Every chest is different — some patients have mostly fat, some have dense glandular tissue, and some also have loose skin. A consultation is the only way to determine what your chest needs.
Will the scar be visible?
Dr. Careaga uses small, strategically placed incisions designed to minimize visible scarring while still allowing precise contouring. Incision placement is tailored to your anatomy and the amount of tissue being addressed.
What is a 'saucer deformity' and how is it avoided?
A saucer deformity is an indented, cratered appearance that can result when too much tissue is removed directly under the areola without contouring the surrounding chest. Dr. Careaga avoids this by treating the chest as a full contouring procedure — preserving a natural transition under the nipple and blending the surrounding tissue for a smooth, uniform result.
What is Renuvion J-Plasma and will I need it?
Renuvion J-Plasma is a technology that tightens the tissue beneath the skin, helping the chest to contract and re-drape more smoothly after gland and fat removal. It is especially useful for patients with mild to moderate loose skin, stretched areolas, or skin that may not shrink well on its own. Not every patient needs it — your anatomy determines whether it's part of the plan.
How long is recovery?
Most patients return to desk work within 7–14 days. Light cardio begins around weeks 2–3. Chest and upper body training resumes around week 4. A compression garment is worn for several weeks to support healing and contour.
When will I see final results?
The chest looks noticeably different relatively quickly, but swelling continues to resolve over several weeks. The final, settled contour becomes fully visible by around 3 months.
Can gynecomastia come back after surgery?
In most cases, results are permanent — particularly for glandular tissue removal. However, significant weight gain, certain medications, or hormonal changes can cause the chest to enlarge again. Maintaining a stable weight and a healthy lifestyle helps preserve results.
Is liposuction alone enough?
For some patients — those whose chest enlargement is primarily fat — liposuction alone may be sufficient. However, dense glandular tissue does not respond to liposuction. Many patients need direct gland excision in addition to liposuction to achieve a flat result. A consultation will determine which approach is right for your anatomy.
Can I have a virtual consultation?
Yes. Virtual consultations are available and allow us to review your concerns, discuss your goals, and evaluate your anatomy based on photos before an in-person visit. This is especially helpful for patients traveling from outside the Miami area.
